End-of-Life Planning

The Best Three Months end-of-life coaching program is an exercise where you imagine you have 90 days to live. This process helps people explore what is most important to them, allowing the development of a nuanced and personalized plan for end-of-life. Through this experience, we examine five domains of life: emotional, spiritual, physical, mental, and practical. For each area of life, you identify your current reality and create action steps that lead to a realistic goal aligned with your values.

This process can be completed at any time. In fact, it is valuable to start the process before an unexpected event occurs. Though you will be prompted to complete legal documents in this process, the document you create through this coaching is non-legal in nature and serves as a valuable map that can be edited as life circumstances change.

These sessions can be completed at your residence or via Zoom

Advance Directives

Five Wishes is a widely used advance care planning document that helps individuals communicate how they want to be cared for if they are unable to make their own medical decisions. This document includes both a medical power of attorney and a living will, enabling individuals to designate who will make healthcare decisions on their behalf and to outline how they wish to be cared for at the end of life. Five Wishes is recognized for being both comprehensive and easy to use. As a Five Wishes facilitator, Abigail’s goal is to help individuals complete the document thoroughly so their wishes are clearly understood and honored to the greatest extent possible.

Five Wishes meets legal requirements for an advance directive in most U.S. states and provides a simple, meaningful way to ensure your preferences are respected.

The Five Wishes Explained

1. Wish One: The Person I Want to Make Care Decisions for Me
This section allows you to choose a Medical Power of Attorney—someone you trust to make medical decisions on your behalf if you become unable to speak for yourself.
2. Wish Two: The Kind of Medical Treatment I Want or Don’t Want
Here, you can outline your preferences for life-prolonging treatments, resuscitation, pain management, and other medical interventions. This ensures your care aligns with your values and goals.
3. Wish Three: How Comfortable I Want to Be
This wish speaks to your personal comfort preferences—pain management, privacy, physical touch, and other aspects of care that affect quality of life.
4. Wish Four: How I Want People to Treat Me
This section captures emotional and relational needs, such as wanting visitors, having loved ones nearby, and being treated with dignity and respect in line with your personal beliefs.
5. Wish Five: What I Want My Loved Ones to Know
Wish Five provides space to express personal messages, end-of-life wishes, forgiveness, and guidance for family and friends. It allows you to share what matters most, bringing peace and clarity to those you care about.
